Subject: Windsurfing Injuries Study

I am an Emergency Physician conducting a Medical study on windsurfing
Injuries and deaths. The study is a questionnaire located at:

It should take five minutes to complete. To date we have over 300 injuries
document. I have a good handle on the deaths in San Fransico Bay
